Job Summary:

Provides expert leadership as part of the specialized Outpatient Parenteral Antibiotic Therapy (OPAT) healthcare team and works with the patient to assure optimal cost effective drug therapy. This includes developing specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient. The clinical specialist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy and works to continuously improve process and practice in the institution to enhance safety of medication therapy.

KEY RESPONSIBILITIES
• Provide and coordinate patient care and clinical drug therapy management services to patients managed with outpatient parenteral antimicrobial therapy (OPAT) and complex (oral) outpatient antimicrobial therapy (CoPAT)

• Obtain and document pertinent information about the patient’s medical condition including signs and symptoms of illness, past and present medications, microbiological data, laboratory data, adverse effects, and medication adherence.

• Ensure the quality, safety, and consistency of clinical Pharmacy services in assigned areas.
• Develops and implements clinical programs designed to enhance patient outcomes and promote the most cost-effective utilization of medications.
• Develops and implements educational programs that advance the professional development, knowledge and effectiveness of staff and clinical pharmacists.
