The Mount Sinai Hospital is seeking a Clinical Pharmacy Manager in Infectious Diseases to join an established Antimicrobial Stewardship Program (ASP) consisting of ID Clinical Pharmacy Managers and physicians. This position will provide the opportunity to function as an ASP pharmacist, working closely with ID and non-ID providers, participate on rounds with the ID consult services, precept pharmacy residents (PGY-1 and PGY-2) and students as well as participate in clinical research, medication use evaluations and institutional committees. 

Description - Anticipated responsibilities include, but are not limited to:

  • Lead and contribute to antimicrobial stewardship initiatives, promoting effective antimicrobial use
  • Offer clinical support to multidisciplinary healthcare teams through rounds as well as providing education on evidence-based antimicrobial therapy and best practices.
  • Review patient treatment plans to ensure optimal antimicrobial therapy, and recommend adjustments to maximize therapeutic benefits while minimizing potential toxicity.
  • Lead the tracking, trending, and analysis of antimicrobial use and resistance patterns
  • participate in education of prescribers, healthcare providers, pharmacists, and nurses to support the stewardship program.
  • Educate prescribers, healthcare providers, pharmacists, and nursing staff to support the antimicrobial stewardship program, while providing counseling to patients and caregivers as needed.
  • Contribute to the development and optimization of medication guidelines, order sets, protocols, and the Electronic Medical Record system to enhance patient care.
  • Initiate and participate in quality improvement projects, recommending updates and modifications as necessary to enhance pharmacy services and patient care.
  • Support the pharmacy department's clinical research initiatives and serve as a preceptor and mentor for PGY1 pharmacy residents, PGY2 pharmacy residents, and pharmacy students from local Colleges of Pharmacy

Qualifications

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy.
  • Active pharmacist license in New York with immunization privileges, or eligible for licensure.
  • Completion of a PGY2 residency in Infectious Diseases or equivalent experience.
  • Board certification in Infectious Disease Pharmacy (BCIDP) preferred.
  • Excellent communication, interpersonal, and teamwork skills.
  • Ability to thrive in a fast-paced environment and effectively prioritize tasks.
