Summary:

  • Provides direct patient care activities by designing, recommending, monitoring, and evaluating patient-specific pharmacotherapy. Serves as the clinical pharmacy expert for a particular service line and is responsible for the training and education of pharmacists/technicians in clinical activities, competency assessment, development and implementation of clinical programs, P&T activities, formulary management, etc.

Schedule:

  • Monday – Friday: 8:00 AM – 4:30 PM
  • Weekends & Holidays: Not Required
  • Key Responsibilities:

    • Deliver comprehensive pharmaceutical care to patients managed by the Infectious Diseases Consult Service and Antimicrobial Stewardship Program.
    • Participate in daily ID consult rounds; assess, monitor, and optimize antimicrobial therapy to ensure cost-effective, evidence-based outcomes.
    • Serve as a subject matter expert for medical staff, the Pharmacy Department, and the Infectious Diseases team.
    • Coordinate pharmacotherapy initiatives and provide educational opportunities for pharmacists and other healthcare professionals.
    • Lead or contribute to quality improvement projects; recommend updates to policies and protocols to enhance pharmacy services.
    • Participate in off-hours on-call rotation to provide infectious diseases pharmacy support as needed.
    • Precept and mentor learners, including pharmacy students and PGY-1 and PGY-2 residents (Infectious Diseases, Critical Care, Emergency Medicine).

Credentials:

  • Education & Experience Requirements:

    • Required:
      • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited College of Pharmacy
      • Completion of a PGY-1 Pharmacy Practice Residency
      • Completion of a PGY-2 Pharmacy Residency in Infectious Diseases
      • Infectious Diseases Pharmacy Specialty Certification (BCIDP)
      • Active Pennsylvania Pharmacist License
