The Ohio State University Wexner Medical Center is looking for a highly motivated pharmacist to join our expanding OPAT pharmacist team. OPAT pharmacists at OSUWMC are integrated in all aspects of patients’ pharmaceutical needs related to OPAT and complex oral antimicrobial therapy (COpAT) as a part of a multidisciplinary team.
Our practice model includes reviewing patients discharging on OPAT/COpAT as part of our ID consult services to ensure accuracy of outpatient regimens, outpatient management including pharmacokinetic and laboratory monitoring, drug-drug interaction review, coordination of outpatient starts and oral transitions/long-acting injectables, and other infusion care coordination with home health, SNF, and other important stakeholders. This position will join our other two OPAT pharmacists and provide team-based coverage, as well as expand coverage to OPAT patients that remain admitted facilitating transitions of care on an every 2-month rotation. This position works M-F, no weekends or holidays. We offer a unique practice model that allows pharmacists to practice at the top of their license in close coordination with our providers.
Beyond providing direct patient care, pharmacists will assume leadership roles in identifying evidence-based opportunities for improvement in patient care and organizational processes, participate in hospital and departmental committees and act as a resource to provide answers to drug information questions. There are numerous opportunities to engage in medication use evaluations and quality improvement projects as well as clinical research.
Teaching involvement may include precepting APPE and pharmacy resident learners, didactic lectures at the College of Pharmacy, and providing educational programs for patients and hospital staff.
