Clinical Pharmacy Specialist - Infectious Diseases / Antimicrobial Stewardship

AHN Forbes Hospital is seeking an Infectious Diseases/Antimicrobial Stewardship (ID/ASP) Pharmacist to join a collaborative clinical pharmacy team. Forbes Hospital is a 300+ bed community hospital serving the eastern suburbs of Pittsburgh area and providing a broad range of acute care and specialty clinical services.  The pharmacist would also serve as a member of the antimicrobial subcommittee for the health network, collaborating with a large team of ID physicians and pharmacists on network initiatives.

Key Responsibilities:

• Serve as the pharmacy expert for Infectious Diseases and Antimicrobial Stewardship, providing consultation and evidence-based recommendations 
• Collaborate with ID physicians and multidisciplinary teams to optimize antimicrobial therapy and improve patient outcomes via prospective audit and feedback, stewardship recommendations, and surveillance activities
• Develop, implement, and maintain antimicrobial treatment guidelines, protocols, clinical pathways, and stewardship initiatives
• Participate in quality improvement, medication use evaluations, outcomes research, and regulatory readiness activities
• Educate pharmacists, physicians, nurses, and other healthcare professionals on infectious diseases pharmacotherapy and antimicrobial stewardship best practices
• Serve as a preceptor for pharmacy residents and pharmacy students
• Participate in local Pharmacy and Therapeutics committees, network antimicrobial subcommittee, and hospital wide initiatives to promote medication safety, quality, and evidence-based patient care

Qualifications:

• PGY2 Infectious Diseases Pharmacy Residency preferred, or
• PGY1 Pharmacy Residency with at least four years of infectious diseases and/or antimicrobial stewardship experience in an acute care setting
